Create and manage custom forms

Nkapio’s form builder lets you create custom forms to gather client information, feedback, consent, and more. Forms can be embedded on your website, shared via link, or printed.

Step 1 — Open the Forms module

From the sidebar, click Forms (or navigate to the Forms section in your menu).

Step 2 — Create a new form

Click the New Form or Create Form button. You’ll enter the form designer where you can add questions and customize the layout.

Step 3 — Add questions and fields

Add questions to your form by clicking Add Question. Choose the question type (text, multiple choice, date, email, phone, checkbox, etc.) and configure options.

Step 4 — Customize and publish

Set form sections, add logic/conditional fields (optional), and then publish your form. Once published, you can embed it on your website or share the form link with clients.
